Acne is an inflammatory disease of the pilosebaceous unit. It is one of the most frequent dermatological pathologies, affecting almost 80% of adolescents between 13-18 years of age.
It represents 25% of Dermatology consultations.
It presents a multifactorial etiology which can last for many years, leave permanent scars and cause important psychological problems in the adolescent who suffers from it.
Nowadays, there are multiple medical and aesthetic treatments to treat acne.
When choosing the appropriate treatment, we must take into account: the type of injury (inflammatory or noninflammatory), the severity and extent of the injury and other possible associated factors.
You can find different types of injuries: non-inflammatory (closed comedones and open comedones or blackheads), superficial inflammatory (papules and pustules), deep inflammatory (nodules and cysts) and residual lesions (hyperpigmentation or scarring)
Non-inflammatory lesions are produced by compacting the follicular cells in the duct, so their appearance improves with exfoliating and keratolytic procedures such as chemical peels and dermabrasion
Inflammatory lesions require either topical or systemic medical treatment depending on their severity and extent. (Antibiotics or isotretinoin)
Residual lesions such as spots and scars, can be treated well with peelings when they are superficial or with lasers when they are deep.
- Chemical peeling and dermabrasion:
Both procedures seek to exfoliate the stratum corneum, which helps to regulate the sebaceous secretion and to unclog the ducts, decreasing the non-inflammatory lesions of acne and giving the skin a less greasy and lighter appearance.
For the peeling, a chemical (glycolic acid) is used, which is applied for a few minutes on the skin after an adequate cleaning, and is subsequently neutralized.
The patient may feel itchiness during the application of the acid and after the treatment redness and sometimes peeling will appear for 24-48h.
The dermabrasion is performed with a diamond tip, being a painless procedure without any side effects for the patient.
- Laser acne scars:
The laser that is used for acne scars is a fractionated laser that creates small localized microscopic lesions that, when regenerated, produce new collagen tissue, improving the appearance of acne marks.
Normally 3-5 sessions are required, which are carried out monthly. The treatment may be slightly painful, so previous topical anesthetic application can be used. After the treatment, the patient will experience redness and slight inflammation for 48-72 hours, but regular life tasks can be done with adequate sun protection.
